The Access Point has been established to process applications for supportive housing and individual mental health supports for youth and adults in the Toronto Area, age 14 years and over.

The Access Point provides streamlined coordinated access to a number of mental health and supportive housing agencies through one application process and provides an ease of access throughout all application stages.

With this form, you can apply for individual support services (like Intensive Case Management, Early Psychosis Intervention or Assertive Community Treatment), Supportive Housing, or both. Please click on the desired program and choose an appointment time to receive a call back.

To apply for services, all applicants must be suffering from serious mental illness, be 16 years of age or older, and must live in the City of Toronto.

Intensive Case Management

One-on-one support to live in the community and be linked to appropriate services by their Case Manager.

Mental Health Supportive Housing

Supportive housing for persons with mental health concerns including addictions. Includes shared housing such as group homes, boarding homes and rooming houses, as well as independent housing.

Assertive Community Treatment Teams

Multi-disciplinary teams provide treatment, rehabilitation and support to people with severe mental illness in their recovery
